Dimagi
Dimagi – the social enterprise behind CommCare and Open Chat Studio, a long-standing Gates Foundation partner whose open-source, offline-first, LLM-enabled platforms for frontline health and development work map directly onto several NAISH use cases.
Dimagi is a US-based social enterprise (founded 2002, headquartered in Cambridge, Massachusetts, with offices in Washington DC, New Delhi, and Cape Town) that builds open-source digital tools for frontline health and development work in low-resource settings. It is a certified B Corporation and a public-benefit corporation, and one of the longest-standing technology partners in global health. It is included in this archive because its platforms – CommCare, Open Chat Studio, CommCare Connect, and SureAdhere – are proven, evidence-backed delivery layers for exactly the frontline, offline-first, multilingual workloads several NAISH use cases require.

Gates Foundation partner. Dimagi received transformational core funding from the Bill & Melinda Gates Foundation to scale CommCare, and in 2023 won a Gates Grand Challenges: Catalyzing Equitable AI grant to build an LLM-powered coach for frontline workers on top of Open Chat Studio (pilot in Malawi). The Gates Foundation is also listed among Open Chat Studio's partners.
Reach
Figures are self-reported by dimagi.com and reflect Dimagi's cumulative portfolio; CommCare now runs as the electronic national community health information system in 9 African countries.
CommCare
CommCare is Dimagi's flagship open-source, no-code platform for building mobile and web apps for frontline data collection, case management, and program delivery. It is designed offline-first for challenging, low-connectivity environments: frontline workers collect data, manage cases, and access job aids without internet, syncing automatically when a connection returns.
Its defining characteristics:
- Offline-first – apps work without connectivity and sync on return, essential for rural and low-connectivity deployments.
- Longitudinal case management – tracks individuals, households, or entities over time and links cases across programs and service points (not just one-off form submissions).
- Real-time reporting – dashboards and reports for program managers.
- Compliance & security – HIPAA, GDPR, and SOC 2; two-factor auth and role-based permissions.
CommCare's unusually strong evidence base – 100+ peer-reviewed publications (including randomised controlled trials) – is rare for a frontline platform.

Open Chat Studio
Open Chat Studio (OCS) is Dimagi's open-source platform for building, evaluating, and deploying LLM-powered chatbots – positioned as "the responsible layer between AI and the people it serves." It targets global-health and social-impact organisations working in low-resource, low-literacy, multilingual, and infrastructure-constrained settings.
Core capabilities:
- Multi-LLM – OpenAI, Anthropic, Google Gemini, Azure, Mistral, LLaMA, Groq, and more.
- Multi-channel deployment – WhatsApp, web widget, SMS, Telegram, Messenger, Slack, voice, email, and CommCare Connect.
- Evaluation tooling – systematic testing of bot behaviour before it reaches vulnerable users.
- RAG / knowledge bases – ground responses in verified local content.

Other platforms
CommCare Connect
A "pay-for-impact" service-delivery marketplace: frontline workers learn skills, deliver verified services, and earn income tied to completed interventions rather than planned activity, with transparent payments to local organisations and workers. Dimagi reports 1.5M+ services delivered; a Uganda pilot enrolled 675+ newborns, ran 1,400 home visits, and disbursed $20,000+ to workers.
SureAdhere
A virtual care platform for medication adherence – video Directly Observed Therapy for tuberculosis and other infectious diseases. Dimagi reports 96% TB treatment completion and a body of publications that have informed CDC and WHO TB treatment guidelines.
Gates Grand Challenges – LLM coach for frontline workers
Dimagi's flagship AI initiative (2023): "Closing the Supervision Gap: An LLM-Powered Coach for Frontline Workers," built on Open Chat Studio and CommCare infrastructure, piloted in Malawi – one of ~50 Gates Catalyzing Equitable AI grants.

Relevance to NAISH
Dimagi's stack is a proven delivery layer for the frontline, offline-first, multilingual, case-management workloads several NAISH use cases require:
- Self-CAIRE digital health coaching – CommCare case management + an Open Chat Studio LLM assistant map directly onto tracking members and health interactions over time.
- Health-record digitisation – CommCare's structured capture and case management complement an OCR/NLP pipeline converting paper records into DHIS2/EHR formats.
- Agricultural advisory – frontline extension-worker apps are a native CommCare pattern; OCS adds a multilingual conversational layer.
Rather than build bespoke frontline apps and chatbots, use-case teams can adopt or integrate Dimagi's evidence-backed, open-source platforms and focus NAISH GPU capacity on the AI/model layer – with a shared Gates Foundation lineage easing partnership.
